Valor pronóstico de la fórmula HUGE en el seguimiento de la enfermedad renal en ancianos

Prognostic value of HUGE formula to follow-up of renal disease in elderly

Introducción

La fórmula hematocrito, urea y género (HUGE) se ha diseñado para el screening de insuficiencia renal. Se considera que si el valor HUGE es mayor que 0, el paciente es portador de insuficiencia renal. En este trabajo analizamos la utilidad de la fórmula HUGE en los pronósticos de la enfermedad renal (fracaso renal agudo [FRA], progresión de la enfermedad renal y mortalidad).

Pacientes y métodos

Se calculó la fórmula HUGE en el periodo basal en 67 ancianos con una edad media de 82,22±6,67 años (rango 69-97), 25 varones (37,3%) y 42 mujeres (62,7%), reclutados aleatoriamente en el año 2006 en consulta externa de Geriatría y de Nefrología General. Tras 36 meses de seguimiento, se analizó la aparición de los pronósticos estudiados y su asociación con el valor HUGE basal.

Resultados

En el periodo basal, el valor medio según fórmula HUGE fue -0,34±4,53 (rango -5,52-19,43) y el de filtrado glomerular estimado por MDRD-4 fue 51,92±17ml/min/1,73m2 (rango 19,18-86,7). Con la fórmula HUGE en el periodo basal, solo 21 pacientes (31,3%) (13 varones y 8 mujeres) tenían un valor mayor que 0. En el seguimiento de los 67 ancianos, 18 pacientes fallecieron. De los 49 pacientes que continuaban con vida a los 36 meses, 12 presentaron algún episodio de FRA. Los pacientes con HUGE > 0 basal presentaron significativamente más FRA y mortalidad (p=0,001), mayor incremento de creatinina sérica a los 36 meses (p=0,003) y un descenso de MDRD de 2,10ml/min/año versus 0,99ml/min/año en paciente con HUGE < 0 (no significativo).

Conclusiones

La fórmula HUGE podría ser de ayuda para identificar el grupo de pacientes con verdadera insuficiencia renal y, por tanto, con riesgo de presentar los pronósticos de la enfermedad renal.

Abstract

Introduction

Hematocrit, urea and gender (HUGE) formula is designed to screen for kidney failure. It is considered that if the HUGE value is greater than 0, the patient was a carrier of kidney failure. This study examines the usefulness of the formula HUGE in prognosis of renal disease (acute renal failure [ARF], renal disease progression and mortality).

Patients and methods

HUGE formula was calculated at baseline in 67 elderly patients with a mean age of 82.22±6.67 years (range 69-97), 25 males (37.3%) and 42 women (62,7%), were recruited at random in our Geriatrics and Nephrology Departments in year 2006. After 36 months of follow-up were analyzed prognosis studied and its association with the basal HUGE value.

Results

At baseline, the mean value HUGE formula was -0,34±4.53 (range -5.5-19.43) and estimated glomerular filtration rate MDRD-4 was 51.92±17ml/min/1.73m2 (range 19.18-86.7). With HUGE formula at baseline, only 21 patients (13 males and 8 females) (31.3%) had a value greater than 0. At follow-up of 67 elders, 18 patients died. Of the 49 patients who were still alive at 36 months, 12 had an episode of ARF. Patients with HUGE > 0 were significantly more ARF and mortality (p=0,001), higher serum creatinine increase at 36 months (p=0,003) and decreased MDRD of 2.10ml/min/year versus 0, 99ml/min/year in patients with HUGE <0 (not significant).

Conclusions

HUGE formula could help to identify the group of patients with renal insufficiency true and therefore at risk of prognosis of renal disease.
